02: I have a question about my ambulance transport? 

The ambulance billing department is available at (623) 465-7400 option 1 Monday through Thursday from 8:00 A.M. to 5:00 P.M., we observe all Federal and State Holidays.

03: Why didn't my insurance pay for the ambulance transport? 

DMFM is required by law to bill for providing an ambulance transport.  You may receive a statement for one or more of the following reasons:

  • We may need to contact you for your insurance information, auto liability, or worker’s compensation information. You may submit your insurance information via this website under "Submit Medical Insurance".
  • We may also need to obtain a signature authorizing us to submit a claim to your insurance on your behalf. You may submit the signature authorization form via this website under "Sign Authorization Form".
  • There may also be a cost-sharing (coinsurance, copay, deductible, etc.) amount that you may be responsible for. If you feel that your insurance has incorrectly processed your claim, please contact your insurance company for further information.

04: How much will an ambulance transport cost? 

Ambulance billing rates are determined by the Department of Health Services (DHS) and can be viewed on the DHS website under Rate Schedule. Select the current month to see current rates of ambulance providers across Arizona, including DMFM.

​The rate of each transportation varies, depending on patient treatments and distance traveled with the patient to the hospital

05: Do you offer a discount if I pay in full? 

Arizona Revised Statute 36-2239 D states “…an ambulance service shall not charge, demand or collect any remuneration for any service greater or less than or different from the rate or charge determined and fixed by the department as the rate or charge for that service.”

As such, it is against the law for DMFM to offer a discount on a ambulance bill.

06: How can I pay my ambulance bill? 

We have multiple ways for you to pay you ambulance bill.  Please have your account number ready, which can be found on your statement.

  •  To make an online payment with a credit card, use the link here:  You can pay your bill online here.
  •  To pay by phone using our secure automated attendant 24/7 please call (623) 257-1139.  
  • To pay by check payable to "Daisy Mountain Fire District" (include your account number) sent to:

               41018 N Daisy Mountain Dr.
               Anthem, AZ, 85086. 

  • You may also call our office to speak to a billing staff member at (623) 465-7400 option 1, Monday through Thursday, 8:00 am to 5:00 pm.
07: I'm experiencing financial difficulty, what are my options? 

Please contact our office at (623) 465-7400 option 1 Monday through Thursday from 8:00 am to 5:00 pm.  

08: Why isn't the cost of the ambulance covered by my taxes? 

Ambulance service providers in Arizona, including ambulances run by city/town fire department, fire districts, or private ambulance companies, have transport fees that are set and regulated by the Arizona Department of Health Services. These fees are separate & distinct from property taxes under the law.
